Keeping older people well at home through innovative models of care

COORDINARE is supporting the innovative models of care that help older people receive timely treatment closer to home, reducing avoidable hospital presentations and supporting residents to remain in familiar surroundings.

Through virtual care initiatives and the Mobile Primary Care After Hours Service Pilot, residential aged care homes across the Goulburn, Upper Lachlan and Illawarra regions have increased access to primary care, after-hours clinical support and specialist advice.

The pilot is demonstrating how use of alternative workforce can assist in managing access to after hours primary care, improving outcomes for both residents and aged care staff.

Highlights include:

Virtual care models implemented in 3 residential aged care homes.

After Hours Service supported 13 homes and 1,000+ aged care beds.

Increased access to primary care and after-hours clinical support.

Reduced self-reported GP fatigue.

Residents can receive care in familiar surroundings, avoiding the distress and disruption often associated with hospital visits, while staff benefit from expert clinical backup and increased confidence in after hours decision-making.
