HealthPathways: Supporting connected care across the region

HealthPathways supported more coordinated care across South Eastern NSW in 2025-26, providing locally relevant clinical guidance and referral pathways that helped improve access to services and patient outcomes.

The ACT and Southern NSW and Illawarra Shoalhaven sites evolved in response to local health priorities and major health system reforms, delivering new and updated pathways across ADHD, asthma, dementia, healthy ageing, menopause, aged care and palliative care.

Highlights include:

Developed two Adult ADHD pathways to support NSW reforms.

Updated 8 asthma pathways with latest evidence.

Expanded NSW Get Healthy Service pathways.

Enhanced palliative care and aged care pathways.

Developed a national My Aged Care pathway.

Strengthened GP access to paediatric support through SUSTAIN.

Improving access to specialist care

HealthPathways supported implementation of NSW's Statewide Referral Criteria, a collaborative initiative bringing together NSW Health, LHDs, specialists, GPs and HealthPathways teams to create more consistent referral processes and improve access to specialist outpatient services.

Within Illawarra Shoalhaven, referral criteria were implemented across:

Diabetes and endocrinology

Gynaecology

Menopause

Maternity

ACT and Southern NSW and Illawarra Shoalhaven HealthPathways supported GP registrars, new graduates and clinicians new to the region by providing local referral pathways, service information and clinical guidance, helping them navigate the health system and connect patients with the right care.
